Insurance
While the physicians of Long Island Behavioral Medicine, PC do not participate in any health insurance plans at this time, we are happy to assist with the paperwork or forms necessary so that patients can seek reimbursement as their plans may allow. Please contact our office staff for any additional information.
For Medicare patients: Please be advised that Long Island Behavioral Medicine, PC has opted out of Medicare. If you choose to see a practitioner at in our practice, you will be required to sign a contract stating that you will not submit to Medicare. If you have a secondary insurance, you may ask for a copy of our Opt-Out Letter to include with your claim submission to your secondary insurance. We can provide you with an insurance form to submit.
